The sustainability of Pos Pelayanan Terpadu (Posyandu) as a community-based health service is strongly influenced by community participation and the social capacity of health cadres. Although Posyandu coverage in Indonesia is relatively high, uneven community involvement remains a major challenge related to weak social relations, trust, and cadre networks. This community engagement program aimed to strengthen the social capital of Posyandu cadres in RW 5 Lembah Sari Village, Pekanbaru City, as a foundation for enhancing community participation and service quality. Using a Participatory Action Research (PAR) approach, cadres and community members were actively involved in problem identification, action planning, implementation, and collective reflection. The program included participatory mentoring, focus group discussions, participatory communication training, and strengthening horizontal and vertical social networks. The results show a strengthening of cadres’ social capital, reflected in improved trust and social interaction, the emergence of collective working patterns, expanded networks with local actors, and increased active community participation in Posyandu activities. These findings indicate that strengthening social capital through participatory approaches is a key strategy for improving the effectiveness and sustainability of community-based health services.
